The Medical Social Worker is a qualified professional that provides skilled social services to patients on an intermittent basis in the home.

My commitment will include:

  • Helping patient and family with end-of-life care issues that encompass personal, financial, and physical decision making.
  • Assisting with placement of patient when end-of-life care may be too cumbersome for the caregiver.
  • Performing services in accordance with the plan of care and are coordinated with other team members.
  • Assessing patients to identify the psychosocial, financial and environmental needs of patients as evidenced by documentation, clinical records, case conferences, team report, call-in logs, and on-site evaluations.
  • Participating in implementation and development of the Plan of Care to ensure quality and continuity of care and proper discharge planning.
  • Assuring the continuity of quality patient care delivered.


  • Education: Master’s degree in social work from a school of social work accredited by the Council of Social Work Education.
  • Licensure: Current state license as a social worker. Must have valid driver’s license.
  • Experience: One (1) year social work experience in a healthcare setting.
  • Skills: Requires excellent verbal and written communication skills with strong interpersonal skills. CPR required.
  • Transportation: Reliable transportation.
