What you’ll do…..

  • Primarily responsible for the timely performance, supervision, training, and documentation of clinical service delivery to your patient and patient’s caregiver in accordance with accepted standards of nursing practice and regulatory guidelines.
  • Report to the RN Clinical Team or Administrator and assist them in improving the clinical and financial performance of the location.
  • Consistently meet staffing productivity standards in accordance with Divisional expectations and shares on-call responsibilities, as scheduled.
  • Conduct yourself in the most professional, legal, and ethical manner with regard to all healthcare and business practices.
  • Comply with all applicable federal and state laws and regulations that govern the healthcare services that it provides.

Job Requirements

What your skills and qualifications are…..

  • A Licensed Practical/Vocational Nurse (LVN) in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
  • 1 - 2 years of experience as a LVN in a healthcare setting.
  • Working knowledge of Medicare/Medicaid home health care benefits, policies and procedures preferred.
  • Possesses a firm foundation of basic nursing skills.
  • Willingness to learn home health regulatory standards related to job-specific duties, including but not limited to: OASIS, Condition of Participation, state-specific licensing rules, diagnosis coding, and internal policies.
  • Willingness to learn and utilize software solution, inclusive of Divisional Work Flow Tasks.
  • Possess a vehicle in good working condition, which is available during work hours, with current proof of auto insurance and a valid driver’s license.
