Job Responsibilities.


As a Home Health Nurse you will administer skilled nursing care to patients requiring professional nursing service in their home. This involves maintaining knowledge of current nursing trends, techniques and innovations so you can best meet the special needs of your patients. You will also be responsible for teaching and supervising the family and other members of the nursing team regarding the best ways to meet each patient’s needs.

Additional responsibilities:

  • Providing leadership in coordinating, promoting and maintaining high standards for the delivery of quality care to the patient by all members of the patient care team
  • Making initial visits, processing orders, notifying physician of patient needs and changes in condition
  • Initiating care, completing certification/recertification orders and discharging summaries
  • Determining the amount and type of nursing needed by each individual patient
  • Initiating appropriate preventive and rehabilitative nursing procedures
  • Providing those services requiring substantial specialized nursing skills
  • Preparing the care plan for the home health aide; and supervising and evaluating the care given by the home health aide as needed
  • Observing signs and symptoms and reporting to the physician and to other agency staff reactions to treatments, including drugs and changes in the patient’s physical or emotional condition or needs
  • Referring Physical Therapist, Speech Therapist, Occupational Therapist, and Medical Social Worker, to patients requiring these specialized skills
  • Following and implementing the agency’s policies and procedures
  • Supervising and teaching other nursing personnel
  • Taking on-call duty nights, weekends and holidays, as assigned
  • Preparing clinical records, care plans, progress notes and summaries of care conferences on patients in a timely manner as per agency policy

Job Requirements

We are looking for a highly organized Registered Nurse with a natural ability to positively motivate others. Strong nursing skills, excellent communication skills, and good planning skills are essential.

Additional requirements:

  • Registered Nurse licensed by the State of Louisiana
  • 1+ years of active experience as a Registered Nurse
  • Good working knowledge of Medicare/Medicaid home health care benefits, policies and procedures, preferred
  • Ability to foster team and continuous quality improvement concepts
  • Firm foundation of basic nursing skills
  • Ability to exercise initiative and independent judgment
