CIMA Hospice is currently in search of an experienced Licensed Social Worker to join their team!  


Primary function is to provide coordinated care in the home to patients in the age groups served by CIMA Hospice and perform psychosocial assessments, analysis, counseling and referrals to meet the needs of the patient and their family/significant others.


Responsibilities include:

  • Improve the patient’s psychosocial condition by intervening in those social, emotional, and environmental aspects of the patient’s life which impact significantly upon his/her overall health status.

  • Performing visits with patients to assess the psychosocial needs of the patients and assist the patient in gaining access to necessary services and community resources.

  • Participates in public information and marketing activities and maintains knowledge of current medical social work trends, techniques and innovations.

  • Upholds regulatory standards and CIMA Hospice policies and performs all duties in accordance with physician’s orders and under the direction and supervision of a Social Service Manager and Director of Nurses.

Job Requirements


1.) Has a Master of Social Work (MSW) degree from a school of social work  accredited by the Council  on Social Work Education (CSWE);        

2.) Must be certified in the State of the Agency location where they will be

Practicing as a Social Worker. 


3.) Must have at least 1 year of experience as a Social Worker in a health care setting. 

4.) Must possess a working knowledge of community resources available to assist patients and their families/significant others as needs are identified.

Must complete CIMA’s on the job internship training with an assigned mentor utilizing agency specific competency modules and will function under the supervision of a mentor until competency has been demonstrated in all appropriate areas.

5.) Must possess a motor vehicle in good working condition, which is available during work hours, with current proof of insurance and a valid Texas Driver’s License.
