Job Description


To provide medically necessary evaluation and treatment of individuals following accepted practice guidelines, in a home health care setting. The Physical Therapist is responsible for evaluating/assessing needs of referred patients and formulating treatment plans. Provides quality therapy services defined in treatment plans. Works cooperatively with physicians, case managers, health care professionals, and other staff members. May supervise physical therapy assistants, aides, and home health aides. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. A certain degree of creativity and latitude is required.

Specific Responsibilities

  • Provides quality therapy services as defined the applicable Standards of Practice.
  • Assists the physician in the functional evaluation of the patient and in the development of the individual plan of treatment, including both short and long-term goals.
  • Develops and implements the therapy component of the patient care plan
  • Provides rehabilitative services
  • Observes and reports to the responsible physician the patient’s progress, reactions to treatments, and any changes in the patient’s condition; modifies the plan of treatment as needed.
  • Instructs the patient, caregiver, and other health team members in methods to improve and correct disabilities
  • Establishes and maintains patient rapport through professional conduct and explanation of treatment plan and goals
  • Communicates regularly with physicians and other health care team members
  • Plans for patient discharge through realistic and attainable long-term goals which may be modified as condition change
  • Prepares the patient and the patient’s caregiver for care after discharge
  • Reports to the Clinical Director any significant patient, family, or physician issues
  • Provides evidence-based clinical care for home health nursing practice utilizing the ANA Scope & Standards of Home Health Nursing Practice

Requirements & Qualifications

  • Current licensure or permit by the applicable state board of therapy.
  • Thorough knowledge of professional theory, practice, techniques, principles and equipment.
  • Effective oral and written communication skills. Ability to document patient care effectively.
  • Ability to establish and maintain a working relationship with patients, other health care professionals, physicians, and the public.
  • A minimum of one (1) year’s experience required.
  • Must hold current state licensure, CPR certification, driver’s license, and automobile liability insurance.

Physical Requirements

Office Work

  • Exerts up to 10 pounds of force occasionally, and or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

Visual Acuity

  • Must have visual ability to prepare and analyze data and figures; transcribe; operate a motor vehicle; read extensively; operate machines, tools and utilized devices.
