Job Description

SUMMARY: The Home Health Aide provides personal care and related services in the home.  He/She functions under the direction, instruction and supervision of the RN Case Manager and the Clinical Supervisor.

  • RESPONSIBILITIES: Tasks to be performed by an HHA must be assigned by and performed under the supervision of an RN who will be responsible for the patient care provided by the HHA. 

  • Helps the patient to maintain good personal hygiene

  • Assists in maintaining a healthful, safe environment

  • Assists the patient with ambulation

  • Assists with certain treatments as ordered by the physician and approved and supervised by the nurse

  • Assists the therapy personnel as needed with rehabilitative processes

  • Encourages the patient to become as independent as possible according to the nursing care plan  

  • Attempts to promote patient's mental alertness through involvement in  activities of interest

  • Gives simple emotional and psychological support to the patient and other members of the household

  • Establishes a relationship with patient and family which transmits trust and confidentiality

  • Prepares a report of his/her visit on the day it is performed and incorporates same in the clinical record          weekly or as directed

  • Reports any change in the patient's mental or physical condition or in his home situation to his/her case manager or to the Clinical Supervisor

  • Carries out his/her assignment as instructed by the nurse or the paramedical team and reports to the nurse when unable to do so

  • Has the emotional and mental maturity necessary for establishing and maintaining a good work relationship with the patient, patient's family, and the personnel of Ross Health Care

  • Works with personnel of other community agencies involved in the patient's care as directed by the staff nurse

  • Performs routine housekeeping tasks as related to a safe and comfortable environment for the patient, as instructed by the professional nurse

  • Receives at least 12 hours of in-service training as required by state and federal regulations

  • Confirms on a weekly basis, the scheduling of visits with the Clinical Supervisor to coordinate necessary visits with other personnel

  • Notifies Ross Health Care of absences due to illness, emergency leave, normal vacation periods, or special professional meetings, which will affect agreed service with Ross Health Care

  • Maintains privacy and adheres to HIPAA policies

  • Complies with all OSHA standards

  • Has the emotional and mental maturity necessary for establishing and maintaining a good work relationship with the patient, patient’s family and the personnel of Ross Health Care

  • Performs skin evaluation every visit and reports abnormal findings to Case Manager/Clinical Supervisor.

  • Perform all other duties as assigned

    TYPICAL PHYSICAL DEMANDS:

  • Able to communicate verbally and in writing to the extent required by the position

  • Able to physically perform the duties required by the position

  • Able to travel to prospective patients' residences

  • Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination

  • Requires standing and walking for extensive periods of time

  • Requires the ability to lift and carry 50 pounds

  • Requires corrected vision and hearing to normal range to record, prepare and communicate appropriate reports

  • Requires working under stressful conditions and working irregular hours

  • Requires some exposure to communicable diseases or body fluids 

  • Physical demands are in excess of those for light duty work

    ORGANIZATIONAL RELATIONSHIPS:

  • Reports to Clinical Supervisor, or other Administrators as assigned.

Job Requirements

QUALIFICATIONS:

  • Completion of the 9th grade; prefer high school graduate                

  • Ability to read and write consistent with job requirements

  • Prefer at least 1 year experience in a home health or hospice setting

  • Has the emotional and mental maturity necessary for establishing and maintaining a good work relationship with the patient, patient's family, and the personnel of Ross Health Care

  • Demonstrates competency in knowledge, skill and ability in order to perform necessary tasks of personal care

  • Current CPR certification required

  • Certified as a home health aide in the State of Oklahoma

  • Effective communication skills, both oral and written

  • Ability to adapt to a variety of personalities and situations

  • Integrity to maintain confidentiality of information

  • Complies with OSHA standards

  • Must have a car with required insurance coverage and a valid state driver's license or reliable transportation
