Job Description

JOB TITLE:                            HOSPICE AIDE



Musthave a compassionate attitude toward the care of terminally ill patients intheir home, or nursing homes. Must be able to read, write, and carry outdirections.  Must demonstrate maturity,and possess the ability to deal effectively with the varying demands of thejob.

Providessupport services, under the direction of the Hospice RN and in accordance withwritten physician orders and Plan of Care established by the IDT for eachpatient.  Duties include assisting thepatient and/or family in the achievement of physical and emotional comfortaccording to the patient’s plan of care through ambulation and exercise, lighthousehold tasks, assistance with ADL, and the performance of basic proceduresas an extension of therapeutic services. Responsible to recognize and report changes in the patient’s conditionsand needs to the Hospice RN.

Job Requirements


Education/Skills                       Highschool graduate or GED

Certification                             Currentstate certification as a nurse assistant is preferred

Training                                   Must have provided at least24 consecutive months of compensated services as a home health or hospice aideor nursing assistant, or completion of an equivalent training program, whichmeets State and Joint Commission training requirements, prior to providingservices.  Must complete CIMA’s on thejob internship training with an assigned mentor utilizing agency specificcompetency modules.  Employee willfunction under the supervision of a mentor until competency has beendemonstrated in all appropriate areas. Must have current CPR.  

Special Requirements             Must possess a motor vehiclein good working condition which is available during work hours, with currentproof of insurance and a valid drivers license, Must demonstrate skillsnecessary to provide care to patients in all age groups.  Must agree to and pass a criminal historybackground check.

Interrelationships                      Hospice physicians, otherhospice staff, hospital and/or nursing home staff, referring physicians, andtheir staff.
