Primaryfunction is to provide coordinated care in the home to patients in the agegroups served by CIMA Hospice and perform psychosocial assessments, analysis,counseling and referrals to meet the needs of the patient and theirfamily/significant others. The Licensed Social Worker (LSW) is toimprove the patient’s psychosocial condition by intervening in those social,emotional, and environmental aspects of the patient’s life which impactsignificantly upon his/her overall health status.  The LSW achieves this goal by performing visitswith patients to assess the psychosocial needs of the patients.  The LSW can then assist the patient ingaining access to necessary services and community resources.  The LSW can also then provide necessaryinterventions and counseling to enhance the patient’s social functioning, toimprove problem-solving skills, and to improve coping abilities.  The LSW participates in public informationand marketing activities and maintains knowledge of current medical social worktrends, techniques and innovations.  TheLSW upholds regulatory standards and CIMA Hospice policies and performs allduties in accordance with physician’s orders and under the direction andsupervision of a Social Service Manager and Director of Nurses.

 Clinical Responsibilities:

  1. Assesses each patient’s social and emotional factors in order to estimate the patient’s capacity and potential to cope with problems of daily living.

  2. Schedules patient visits as necessary according to patient needs and as per physician’s orders.

  3. Helps each patient and his/her family to understand, accept and follow medical recommendations.

  4. Provides services to each patient aimed at restoring the patient to optimal psychological and social adjustment.

  5. Assists each patient and his/her family with personal and environmental difficulties.

  6. Utilizes community resources to assist each patient in resuming life in the community or in learning to live within the limitations of his/her disability or illness.

  7. Assists the physician and other members of the care team in understanding the significant social and emotional factors related to each patient’s health problems.

  8. Coordinates with all members of each patient’s care team regarding changes in the patient’s condition and needs.

  9. Ensures that each patient’s care plan includes identified psychosocial problems, approaches and goals.

  10. Follows and implements the agency’s policies and procedures regarding infection control and safety measures.

  11. General job responsibilities summary include:

  1. Ongoing interdisciplinary assessment of the patient;

  2. Providing services that are ordered by the physician as indicated in the plan of care;

  3. Patient, caregiver, and family counseling;

  4. Patient and caregiver education;

  5. Preparing clinical notes;

  6. Communication with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care;

  7. Participation in the Agency’s QAPI program; and

  8. Participation in Agency’s-sponsored in-service training

Job Requirements



1.) Has aMaster of Social Work (MSW) degree from a school of social work  accredited by the Council  on Social Work Education; or

2.)Has aBaccalaureate degree in Social Work from an institution accredited by the Councilon Social Work Education, or a baccalaureate degree in Psychology, Sociology,or other field related to Social work is supervised by a MSW and has 1 yearsocial work experience in a healthcare setting; or

3.) Has aBaccalaureate degree from a school of Social Work accredited by the Council onSocial Work Education, is employed by the Hospice Agency before December 2,2008, and is not required to be supervised by a MSW.



Must becertified in the State of the Agency location where they will be

Practicing as  aSocial Worker.  Must have a current TexasDriver’s License.


Must have at least one yearof experience as a Social Worker in a

health care setting.  Must possessa working knowledge of community resources available to

assist patients and their families/significant others as needs areidentified. Must complete CIMA’s

on the job internship training with an assigned mentor utilizing agencyspecific competency modules.

Employee will function under the supervision of a mentor until competencyhas been demonstrated

in all appropriate areas.


Strong leadership and organizational skills.  Excellent verbal and written communicationskills.  Exhibition of good judgment andability to work independently. May be required to work under stressfulconditions to meet deadlines and to make quick decisions and resourceacquisition in order to meet patient/family/significant others psychosocialneeds. Must possess a motor vehicle in good working condition, which isavailable during work hours, with current proof of insurance.  Must agree to and pass a criminal historybackground check.
