What you’ll do…..

  • Performing face-to-face encounters for hospice patients, ages 16 years and older, according to State and Federal regulations and accepted Standards of Practice.

  • Conducts physical assessments of patients as part of the recertification of terminal illness process, per agency policy

  • Observing signs and symptoms and reporting to the physician reactions to treatments, including drugs, and changes in the patient's physical or emotional condition

  • Teaching, supervising and counseling the patient and caregivers regarding the nursing care needs and other related problems of the patient at home

  • Conduct yourself in the most professional, legal, and ethical manner with regard to all healthcare and business practices.

  • Comply with all applicable federal and state laws and regulations that govern the healthcare services that it provides.

Job Requirements

What your skills and qualifications are…..

  • Certified Nurse Practitioner in the state of Texas, or in accordance with the Board of Nurse Examiners rules for Nurse Practitioner Licensure Compact. 

  • At least one year of Nurse Practitioner experience is preferred. 

  • Must complete 20 contact hours of continuing education in the advanced practice role and population focus area or renewal of the certification as specified every two (2) years coinciding with the nurse’s license renewal. 

  • An APRN who has Prescriptive Authority must also complete at least five additional CEs in pharmacotherapeutics within the two years preceding license renewal.  Must complete CIMA’s agency specific orientation.  

  • Excellent verbal and written communication skills.  Exhibition of good physical stamina, as well as the ability to be self directed and work with little supervision.

  • Must possess a motor vehicle in reliable working condition, which is available during work hours.  Must provide and maintain current proof of insurance in accordance with the state requirements and possess a valid drivers license.  Must agree to and pass a criminal history background check.
