What you’ll do….
 



  • Assume primary responsibility for directing and providing the nursing care for hospice patients in their place of residence, ages 16 years and older.  



  • Coordinate Care and maintain regular communication with members of the interdisciplinary team, alerting attending physician and assigned Case Manager to any changes in the patient’s condition or needs as well as potential problems or issues.





  • Participate in On- call after hours and weekend rotations; Participates in weekend and holiday coverage as necessary.



  • Promotes hospice philosophy and Supports agency and corporate policies, goals, and objectives.

What your skills and qualifications are…..



  • A registered nurse licensed in the state of work, or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. 

  • Two years’ experience as a Registered Nurse in a clinical setting, hospice preferred. 

  • Must complete a minimum of 20 hours of continuing education within each 24- month licensure period. 

  • Strong leadership and organizational skills. 

  • Excellent verbal and written communication skills.

  • Must possess a motor vehicle in good working condition, which is available during work hours, with current proof of insurance.
