ESSENTIAL FUNCTIONS: 



  • The RN Case Manager is responsible for directing and providing the nursing care for hospice patients in their place of residence, ages 16 years and older.  



  • Coordinate care with the assigned interdisciplinary team, patient/family and referring agency.



  • Supports agency and corporate policies, goals, and objectives.  

  • Promotes a positive working relationship between agency and corporate personnel.

  • Attends and participates in staff meetings, educational programs and community events, as requested.

  • Promotes hospice philosophy.

  • Promotes teamwork by participating actively and appropriately as member of Interdisciplinary team.

  • Completes admission process according to agency/corporate policy.

  • Collaborates with IDT members regarding initial plan of care, patient’s condition and other essential information.

  • Participates in On- call after hours and weekend rotations

  • Participates in weekend and holiday coverage as necessary.

QUALIFICATIONS:





  • Must be a registered nurse licensed in the state of work, or in accordance with the  Board of Nurse Examiners rules for Nurse Licensure Compact. 



  • Two years’ experience as a Registered Nurse in a clinical setting, hospice preferred. 



  • Must complete a minimum of 20 hours of continuing education within each 24- month licensure period. 



  • Strong leadership and organizational skills. 



  • Excellent verbal and written communication skills.



  • Must possess a motor vehicle in good working condition, which is available during work hours, with current proof of insurance.
