ESSENTIAL FUNCTIONS: 

    • The RN Case Manager is responsible for directing and providing the nursing care for hospice patients in their place of residence, ages 16 years and older.  

    • Coordinate care with the assigned interdisciplinary team, patient/family and referring agency.

    • Supports agency and corporate policies, goals, and objectives.  
    • Promotes a positive working relationship between agency and corporate personnel.
    • Attends and participates in staff meetings, educational programs and community events, as requested.
    • Promotes hospice philosophy.
    • Communicates identified needs and potential solutions to IDG team, referring provider and medical director.
    • Explains hospice services and benefits to patients, family members and significant others.
    • Initiates initial plan of care with authorization of the attending physician and hospice medical director.
    • Makes appropriate referrals for other discipline services, medical equipment and pharmaceutical needs, i.e., pain/symptom management, DME, meds, supplies, services.
    • Promotes teamwork by participating actively and appropriately as member of Interdisciplinary team.
    • Completes admission process according to agency/corporate policy.
    • Collaborates with IDT members regarding initial plan of care, patient’s condition and other essential information.
    • Participates in On- call after hours and weekend rotations
    • Participates in weekend and holiday coverage as necessary.
    • Performs other duties as assigned.   



Job Requirements

QUALIFICATIONS:

Education/Skills

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing Certification/Licensure          Must be a registered nurse licensed in the state of work, or in accordance with the  Board of Nurse Examiners rules for Nurse Licensure Compact.  Must have a current  Drivers License.

Training

Two years experience as a Registered Nurse in a clinical setting, hospice preferred.  Must complete a minimum of 20 hours of continuing education within each 24- month licensure period.  Must complete CIMA’s on the job internship training with an assigned mentor utilizing agency specific competency modules.  Employee will function under the supervision of a mentor until competency has been demonstrated in all appropriate areas.  

Special Requirements

 Strong leadership and organizational skills.  Excellent verbal and written communication skills.  Exhibition of good judgment and ability to work independently. Must possess a motor vehicle in good working condition, which is available during work hours, with current proof of insurance.  Must agree to and pass a criminal history background check.
