ESSENTIAL FUNCTIONS:



Primary function is to provide coordinated care in the home to patients in the age groups served by CIMA Hospice and perform psychosocial assessments, analysis, counseling and referrals to meet the needs of the patient and their family/significant others. The Licensed Social Worker (LSW) is to improve the patient’s psychosocial condition by intervening in those social, emotional, and environmental aspects of the patient’s life which impact significantly upon his/her overall health status. The LSW achieves this goal by performing visits with patients to assess the psychosocial needs of the patients. The LSW can then assist the patient in gaining access to necessary services and community resources. The LSW can also then provide necessary interventions and counseling to enhance the patient’s social functioning, to improve problem-solving skills, and to improve coping abilities. The LSW participates in public information and marketing activities and maintains knowledge of current medical social work trends, techniques and innovations. The LSW upholds regulatory standards and CIMA Hospice policies and performs all duties in accordance with physician’s orders and under the direction and supervision of a Social Service Manager and Director of Nurses.





Clinical Responsibilities:



  • Assesses each patient’s social and emotional factors in order to estimate the patient’s capacity and potential to cope with problems of daily living.

  • Schedules patient visits as necessary according to patient needs and as per physician’s orders.

  • Helps each patient and his/her family to understand, accept and follow medical recommendations.

  • Provides services to each patient aimed at restoring the patient to optimal psychological and social adjustment.

  • Assists each patient and his/her family with personal and environmental difficulties.

  • Utilizes community resources to assist each patient in resuming life in the community or in learning to live within the limitations of his/her disability or illness.

  • Assists the physician and other members of the care team in understanding the significant social and emotional factors related to each patient’s health problems.

  • Coordinates with all members of each patient’s care team regarding changes in the patient’s condition and needs.

  • Ensures that each patient’s care plan includes identified psychosocial problems, approaches and goals.

  • Follows and implements the agency’s policies and procedures regarding infection control and safety measures.

  • General job responsibilities summary include:




  1. Ongoing interdisciplinary assessment of the patient;



  2. Providing services that are ordered by the physician as indicated in the plan of care;



  3. Patient, caregiver, and family counseling;



  4. Patient and caregiver education;



  5. Preparing clinical notes;



  6. Communication with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care;



  7. Participation in the Agency’s QAPI program; and



  8. Participation in Agency’s-sponsored in-service training



Job Requirements

QUALIFICATIONS:





Education/Skills



1.) Has a Master of Social Work (MSW) degree from a school of social work  accredited by the Council  on Social Work Education; or



2.)Has a Baccalaureate degree in Social Work from an institution accredited by the Council on Social Work Education, or a baccalaureate degree in Psychology, Sociology, or other field related to Social work is supervised by a MSW and has 1 year social work experience in a healthcare setting; or



3.) Has a Baccalaureate degree from a school of Social Work accredited by the Council on Social Work Education, is employed by the Hospice Agency before December 2, 2008, and is not required to be supervised by a MSW.



 





Certification/Licensure         



Must be certified in the State of the Agency location where they will be



Practicing as  a Social Worker.  Must have a current Texas Driver’s License.





Training                                 



Must have at least one year of experience as a Social Worker in a health care setting.  Must possess a working knowledge of community resources available to assist patients and their families/significant others as needs are identified. Must complete CIMA’s on the job internship training with an assigned mentor utilizing agency specific competency modules.



Employee will function under the supervision of a mentor until competency has been demonstrated in all appropriate areas.





Special Requirements



Strong leadership and organizational skills.  Excellent verbal and written communication skills.  Exhibition of good judgment and ability to work independently. May be required to work under stressful conditions to meet deadlines and to make quick decisions and resource acquisition in order to meet patient/family/significant others psychosocial needs. Must possess a motor vehicle in good working condition, which is available during work hours, with current proof of insurance.  Must agree to and pass a criminal history background check.
