Job Description

POSITION:                   LICENSED SOCIAL WORKER





ORGANIZATION:         Jordan Health Services - Medicare





REPORTS TO:             ADON





POSITION SUMMARY:





The Licensed Social Worker (LSW) is to improve the patient’s psychosocial condition by intervening in those social, emotional, and environmental aspects of the patient’s life which impact significantly upon his/her overall health status.  The LSW achieves this goal by performing home visits with patients to assess the psychosocial needs of the patients.  The LSW can then assist the patient in gaining access to necessary services and community resources.  The LSW can also then provide necessary interventions and counseling to enhance the patient’s social functioning, to improve problem-solving skills, and to improve coping abilities.  The LSW participates in public information and marketing activities and maintains knowledge of current medical social work trends, techniques and innovations.  The LSW upholds regulatory standards and Jordan Health Services (JHS) policies and performs all duties in accordance with physician’s orders and under the direction and supervision of a Masters Level Social Worker and the Assistant Director of Nurses.
Clinical and Administrative duties include:



  1. Ongoing interdisciplinary assessment of the patient;



  2. Providing services that are ordered by the physician as indicated in the plan of care;



  3. Patient, caregiver, and family counseling;



  4. Patient and caregiver education;



  5. Preparing clinical notes;



  6. Communication with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care;



  7. Participation in the Home Health Agency’s QAPI program; and



  8. Participation in Home Health Agency’s-sponsored in-service training





Job Requirements

KNOWLEDGE, SKILLS, ABILITIES, AND OTHER CHARACTERISTICS:





I.          Education






  1. A social worker must have a master’s or doctoral degree from a school of social work accredited by the Council Completion of a Master’s degree in social work required ( as per Sec. 484.115(m))




    II.         Experience







  1. Minimum of one (1) year’s social work experience in a health care setting ( as per Sec. 484.115(m))



  2. Working knowledge of Medicare/Medicaid home health care benefits, rules and regulations preferred.



    III.        Skills & Abilities







  1. Have the ability to motivate others in a positive manner.



  2. Have good verbal and written communication skills.



  3. Possess well developed interpersonal skills.



  4. Have the ability to organize work and schedule effectively.



  5. Possess a firm foundation of medical social work skills.



  6. Be willing and able to foster the team and continuous quality improvement concepts.

                1.         General







  1. Must have satisfactory references from previous employers.



  2. Must be able to pass a criminal history check.



  3. Must be able to pass a random drug test.



  4. Must have a dependable vehicle and a valid driver’s license.
