Job Description

Licensed Practical Nurse

 

SUMMARY: The Licensed Practical Nurse(LPN) provides nursing care and teaching to patients and families.  He/She also functions as an assistant to thephysician and registered nurse, and is supervised by the RN, the Home HealthClinical Supervisor, or other administrator as assigned.

QUALIFICATIONS:


•       Graduate of an approved program ofpractical/vocational nursing


•       Licensed as a licensed graduate practical nursein the State of Oklahoma


•       One year of experience as a nurse preferred


•       Current CPR certification required


•       Must have a car with required insurance coverageand a valid state driver's license or reliable transportation

RESPONSIBILITIES:


•       Participates in the planning and coordination oftotal patient care in conjunction with the RN and the physician's plan oftreatment


•       Follows the nursing care plan for each assignedpatient


•       Observes the patient; evaluates the care of thepatient; applies his/her knowledge of nursing skills to the patients to whomhe/she is assigned


•       Accurately reports and records the patient'scondition and care, including signs and symptoms which may be indicative ofchange


•       Prepares clinical/progress notes on the day ofthe visit and incorporates it in the clinical record


•       Assists the patient with the activities of dailyliving while teaching appropriate self-care techniques


•       Provides and maintains a safe environment forthe patient


•       Provides optimum physical and emotionalenvironment


•       Assists the physician and registered nurse incomplex nursing situations


•       Participates in case conference, in-service, andother programs and meetings as needed.


•       Performs within limits of preparation andexperience


•       Confirm, on a weekly basis, the scheduling ofvisits with the nurse to coordinate necessary visits with other personnel


•       Perform treatments and specialized procedures asassigned per physician's plan of care including preparation of equipment andmaterials for treatments observing aseptic technique as required


•       Demonstrates competency/experience signed by anRN or specific training program director


•       Maintains privacy and adheres to all HIPAApolicies


•       Complies with OSHA standards


•       Perform all other duties as assigned

TYPICAL PHYSICAL DEMANDS:


•       Able to communicate verbally and in writing tothe extent required by the position


•       Able to physically perform the duties requiredby the position


•       Able to travel to prospective patients'residences


•       Requires full range of body motion includinghandling and lifting patients, manual and finger dexterity and eye-handcoordination 


•       Requires standing and walking for extensiveperiods of time


•       Requires the ability to lift and carry 50 pounds 


•       Requires corrected vision and hearing to normalrange to record, prepare and communicate appropriate reports 


•       Requires working under stressful conditions andworking irregular hours


•       Requires some exposure to communicable diseasesor body fluids 


•       Physical demands are in excess of those forlight duty work


•       Has the emotional and mental maturity necessaryfor establishing and maintaining a good work relationship with the patient,patient’s family, and the personnel of Ross Health Care

 

ORGANIZATIONAL RELATIONSHIP:


•       Reports to Clinical Supervisor, RN Case Manager,or other Administrators as assigned
