Job Description

Staff Licensed Practical Nurse (LPN)

Responsible To: CHC/Branch Supervisor/Private Duty Services Supervisor

Qualifications:

1. Must be a graduate of a state approved School of Practical Nursing

2. Must have a current license in the State of Oklahoma to practice as an LPN.

3. CPR Certification

4. Must have recent experience in skilled nursing practice of at least one (1) year.

5. Must be in good physical and emotional health.

6. Must be able to lift 50 lbs., be able to stoop/bend/walk without aid and have normal hearing/vision/speech capabilities

Functions:

1. Provide direct skilled home nursing care as directed by the physician’s orders.

2. Identify patient’s needs and assist the RN in the preparation of the Nursing Care Plan. Update the care plans and teaching plans as needed.

3. Perform treatments and administer medications as prescribed by the physician in accordance with the Oklahoma State Nurse Practice Act.

4. Assess effectiveness of treatments and monitor the patient’s response.

5. Maintain a clean and safe environment.

6. Maintain complete and accurate clinical records on all patients assigned to her/his care.

7. Report significant findings or changes in the patient’s condition to the RN or physician.

8. Institute emergency procedures to offset life-threatening situations when ordered (CPR).

9. Attend regular staff, case conferences meetings and in-services.

10. Submit paper work within the time frame allotted.

11. Assess the physical, psychological, social and environmental status of the patient and report changes in status to the physician.

12. Work with other service to provide a continuity of care and to evaluate the care given.

13. Support agency policies and assist in the process of policy changes.

14. Participate in all activities relevant to the professional service provided.

15. Maintain program compliance with applicable laws and regulations.

16. Assume any other duties as designated by management.
