Job Description


Musthave a compassionate attitude toward the care of terminally ill patients intheir home, or nursing homes. Must be able to read, write, and carry outdirections.  Must demonstrate maturity,and possess the ability to deal effectively with the varying demands of thejob.

Actas a liaison for CIMA Hospice to the patient and their family upon admission toassure the admission process was performed timely and professionally and metthe needs of the patient and their family members/significant others.  Assist in obtaining clinical documents fromother healthcare providers to facilitate the determination of appropriatenessfor admission to hospice.  Coordinate andcompile data for the patient and family/caregiver satisfaction surveys.

Job Requirements


Education/Skills                       Highschool graduate or GED

Training                                   Prior healthcare customerservice experience preferred.  Mustcomplete CIMA’s on the job internship training program with an assigned mentor utilizingagency specific competency modules. Employee will function under the supervision of a mentor untilcompetency has been demonstrated in all appropriate areas.   

Special Requirements             Must possess a motor vehiclein good working condition which is available during work hours, with currentproof of insurance and a valid driver’s license.  Must possess professional interpersonalcommunication skills, and be highly organized and self-motivated.  Must be able to interact effectively withpatients in all age groups served by CIMA Hospice.  Must agree to and pass a criminal historybackground check.

Interrelationships                      Hospice physicians, Administrator,Director of Clinical Services, Provider Relations Executives, other hospicestaff, hospital and/or nursing home staff, referring physicians and their staff,patients, and patient’s family/significant others.
