Job Description

To provide medically necessary evaluation and treatment of individuals following accepted practice guidelines, in a home health care setting. The Physical Therapy Assistant is responsible for evaluating/assessing needs of referred patients and formulating treatment plans. Provides quality therapy services defined in treatment plans. Works cooperatively with physicians, case managers, health care professionals, and other staff members. May supervise home health aides. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. A certain degree of creativity and latitude is required.

Specific Responsibilities

 Provides quality therapy services as defined the applicable Standards of Practice.

 Assists the physician in the functional evaluation of the patient and in the development of the individual plan of treatment, including both short and long-term goals.

 Develops and implements the therapy component of the patient care plan

 Provides rehabilitative services

 Observes records and reports to the physical therapist and other members of the patient’s health care team, the patients reactions to treatments, treatment program and any changes in the patient’s condition; modifies the plan of treatment as needed.

 Instructs the patient, caregiver, and other health team members in methods to improve and correct disabilities

 Establishes and maintains patient rapport through professional conduct and explanation of treatment plan and goals

 Communicates regularly with physicians and other health care team members

 Plans for patient discharge through realistic and attainable long-term goals which may be modified as condition change

 Prepares the patient and the patient’s caregiver for care after discharge

 Reports to the Agency Director any significant patient, family, or physician issues

 Provides evidence-based clinical care for home health nursing practice utilizing the ANA Scope & Standards of Home Health Nursing Practice

  •  Current licensure or permit by the applicable state board of therapy.
  •  Thorough knowledge of professional theory, practice, techniques, principles and     equipment.
  •  Effective oral and written communication skills. Ability to document patient care effectively.
  •  Ability to establish and maintain a working relationship with patients, other health care professionals, physicians, and the public.
  •  A minimum of one (1) year’s experience required.
  •  Must hold current state licensure, CPR certification and driver’s license.
