Job Description

The Registered Nurse (RN) administers skilled nursing care to patients requiring professional nursing service in the patient’s home.  He/she maintains knowledge of current nursing trends, techniques and innovations to meet the special needs of his/her patients.  The RN teaches and supervises the family and other members of the nursing team regarding the best ways to meet each patient’s needs.  He/she also upholds company and regulatory standards of nursing practices and analyzes and evaluates the services provided to improve the quality of care.  The Registered Nurse performs all duties in accordance with physician’s orders and under the direction and supervision of the Assistant Director of Nurses.

Key Accountabilities

  • Makes the initial evaluation visit.
  • Develops and implements a written plan of care and obtains physician authorization to cover the total scope of home care following a thorough initial assessment of the patient.
  • Assess the physical, psychological, social and environment status of the patient and report changes in the state to the physician
  • Administer skilled nursing care to the patient in the home as ordered by physician and as follows the Oklahoma State Nurse Practice Act.
  • Counsels the patient and family members regarding care needs of the patient and other problems related to the patient at home.
  • Develops realistic goals for patient care and rehabilitation with input from the patient and caregivers.  Re-evaluate and re-defines goals on an ongoing basis according to patents needs and progress.
  • Maintains complete and accurate clinical records on all patients assigned to his/her care.
  • Make assignments to personnel with specific instructions as to type and extent of care to be provided.
  • Makes supervisory visits, as required.
  • Works with other services to provide a continuity of care and to evaluate the care given.
  • Responsible to taking calls as assigned and reacting to calls in a prompt prudent manner.
  • Supports agency policies and assists in the process of policy change.
  • Participates in all activities relevant to the professional serve provided.
  • Responsible in keeping items needed for employee file up to date (ex. Verification of Insurance).
  • Maintains program compliance with applicable laws and regulations.
  • Responsible for attending scheduled in-services, case conferences and staff meetings.
  • Submits paperwork in a timely manner as specified by policy and procedures.
  • Ensures adequate supplies; which pertain to the individual patent are stocked at a patients home.
  • Assume any other duties as designated by management.

Job Requirements

Education/Experience:

  1. Must be a graduate of an accredited school of nursing with a current license to practice nursing in the state of Oklahoma.
  2. Completion of a Baccalaureate or Masters degree in nursing or related field preferred.


Skills and Abilities:

  1. Advantage Program Certification.
  2. Travel an average of 80 to 90 miles per day.


Other:

  1. Have good verbal and written communication skills.
  2. Must be able to pass a criminal history check.
  3. Must be able to pass a random drug test.
  4. Must have a dependable vehicle and a valid driver’s license.
