Job Description

Staff Registered Nurse (RN)

Responsible To: Branch/CHC Supervisor/Private Duty Services Supervisor

Qualifications:

1. Must be a graduate of an accredited school of nursing with a current license to practice nursing in the state of Oklahoma.

2. CPR Certification

3. Must be able to lift 50 lbs, be able to stoop/bend/walk without aid, have normal hearing/vision/speech capabilities, and be in good physical and emotional health with no communicable diseases.

Functions:

1. Makes the initial evaluation visit.

2. Develops and implements a written plan of care and obtains physician authorization to cover the total scope of home care following a thorough initial assessment of the patient.

3. Assess the physical, psychological, social and environment status of the patient and report changes in state to the physician.

4. Administer skilled nursing care to the patient in the home as ordered by physician and as follows the Oklahoma State Nurse Practice Act

5. Counsels the patient and family members regarding care needs of the patient and other problems related to the patient at home.

6. Develops realistic goals for patient care and rehabilitation with input from the patient and caregivers. Re-evaluate and re-defines goals on an ongoing basis according to patients needs and progress.

7. Maintains complete and accurate clinical records on all patients assigned to his/her care.

8. Make assignments to personnel with specific instructions as to type and extent of care to be provided.

9. Makes supervisory visits, as required.

10. Works with other services to provide a continuity of care and to evaluate the care given.

11. Responsible to taking calls as assigned and reacting to calls in a prompt prudent manner.

12. Supports agency policies and assist in the process of policy change.

13. Participates in all activities relevant to the professional service provided.

14. Responsible in keeping items needed for employee file up to date (ex. Verification of insurance)

15. Maintains program compliance with applicable laws and regulations.

16. Responsible for attending scheduled in-services, case conferences and staff meetings.

17. Submits paperwork in a timely manner as specified by policy and procedures.

18. Ensures adequate supplies; which pertain to the individual patient are stocked at a patient’s home.

19. Assume any other duties as designated by management.
