Job Description

POSITION:      ClinicalTeam Manager                

ORGANIZATION:     Jordan Health Services - Medicare     

REPORTS TO:   Administrator/DON           


            The Clinical Team Manager (CTM) directs and coordinatesskilled nursing services, therapeutic services and administrative             services for employees/patients onhis/her own specified teams, coordinating with other CTM’s andAdministrator/DON to promote a positive,successful work environment.   



I.           Agency Operations                                                            (component proportion   75%)

  1. Completes assigned Work Flow tasks within timeframes in accordance with Divisional expectations.
  2. Oversees subordinates to ensure their assigned Work Flow tasks are consistently completed within timeframes in accordance with Divisional expectations.
  3. Leads case conferences, participates in the process to either recertify or discharge an individual from service.
  4. Assists in the review and approval of clinical documentation related to admission, recertification, discharge, including but not limited to OASIS data sets, plans of care, add on evaluation orders and documentation.
  5. Performs skilled nursing visits as needed.
  6. Generates monthly on-call schedule and shares on-call responsibilities if needed.
  7. Reviews weekly billing reports with Medical Records Coordinator and takes necessary action to resolve outstanding claims/issues.
  8. Reviews billing and scheduling reports as well at month end. 
  9. Reviews all necessary reports at least twice weekly (visit time audit report, worker productivity report, agent summary report, missed visit report, as well as other administrative reports) and takes appropriate action as necessary to ensure all provided services and programs improve coordination, quality, and continuity of patient care.
  10. Reports significant patient findings to Administer and collaborates on actions or interventions to be taken.
  11. Process client occurrence events, infection control events, and client complaints, ensuring adequate investigation, follow-up and intervention have been performed.
  12. Orders supplies for office stock.  Reviews designated orders submitted by team members.  Changes orders if appropriate and gives final approval before order to be shipped.
  13. Approve time sheets of their team for Payroll.
  14. Participates as a member of the QAPI Committee
  15. Job Responsibilities as aligned with HH Condition of Participation 42 C.F.R. 484.75(b) published in Federal Register


  1. Ongoing interdisciplinary assessment of the patient;

  2. Providing services that are ordered by the physician as indicated in the plan of care;

  3. Patient, caregiver, and family counseling;

  4. Patient and caregiver education;

  5. Preparing clinical notes;

  6. Communication with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care;

  7. Participation in the Home Health Agency’s QAPI program; and

  8. Participation in Home Health Agency’s-sponsored in-service training

    II.           Staffing / Staff Development                                                            (component proportion   25%)

  1. Conducts, attends and/or participates in staff meetings, client care conferences and in services as necessary.
  2. Conducts timely performance evaluations on subordinates to also be reviewed and signed by Administrator.
  3. Evaluated own job performance and utilization of resources in planning for professional growth.
  4. Understands, adheres to and performs all duties in accordance with established agency policies and procedures.
  5. Follows and implement the agency’s policies and procedures regarding infection control and safety measures.
  6. Maintains availability at all times during his/her schedules work hours, either in person or by telecommunications.
  7. Collaborates with member of marketing team to assist in identifying potential referral sources and patients appropriate for home care.
  8. Maintains current knowledge of nursing quality improvement practice as pertinent to the job and is responsible for his/her own continuing education.
  9. Works to foster a team atmosphere and maintain a team attitude.

Job Requirements


I.            Education and Experience

  1. Must be a Registered Nurse (RN) licensed in the state of Texas
  2. In accordance with licensing regulations, must meet the following criteria:
    1. For an agency without a home dialysis designation, a supervising nurse and alternate supervising nurse must each be a registered nurse (RN) licensed in the state of  licensure or in accordance with the Board of Nursing Examiners rules for Nurse Licensure Compact (NLC) and have at least one year experience as an RN within the last 36 months.

  1. Minimum of three (3) years of active experience as a RN, preferably with at least (1) year in home health.
  2. Certification in OASIS, preferred.   If not certified at the time of hire, willing to obtain training and education, as available, to take exam within 18 months of employment.
  3. Possession of a working knowledge of Medicare/Medicaid home health care benefits, policies and procedures, preferred.

II.          Skills & Abilities

  1. Has a positive attitude and willingness to learn throughout employment.
  2. Self-starter with excellent verbal and written communication skills.
  3. Have the ability to motivate others in a positive manner
  4. Possess good interpersonal skills, able to work with both clinical and non-clinical staff members.
  5. Willingness to learn and utilize software solution, inclusive of Divisional Work Flow tasks.
  6. Must be organized and have the ability to manage multiple tasks & priorities at any given time.
  7. Possesses a firm foundation of basic nursing skills.
  8. Willingness to learn home health regulatory standards related to job-specific duties, including but not limited to: OASIS, Conditions of Participation, State-specific licensing rules, diagnosis coding, and internal policies.

III.        Other

  1. Must have satisfactory references from previous employers.
  2. Must be able to pass a criminal history check.
  3. Must be able to pass a random drug test.
  4. Must have a dependable vehicle and a valid driver’s license.
