JOB TITLE:                                           On-Call Nurse (RN)

EVALUATED BY:                                ADMINISTRATORAND/OR DIRECTOR OF NURSING

       


ESSENTIAL FUNCTIONS:

Summary: provision of on-site skilled nursing triage, assessment,planning and provision of patient care in order to maximize the palliation ofsymptoms when patient care needs arise after hours. Travel to patient homesrequired with mileage reimbursement. 

Schedule: On Call from 5:00pm to 8:00am; Monday – Thursday each weekand 5:00pm Friday to 8:00am Monday every other weekend.

JOB DUTIES/KNOWLEDGE

  • Assumes responsibility upon dispatch for after-hour requests for on-site support.  Evaluates and determines the need for clinical and other services including assessments, symptom management and care plan updates, documenting in (EMR) and coordinating these with appropriate Hospice staff members.

  • Obtains data on current or potential physical, psychological, social and spiritual factors that may impact patient and family’s needs and initiate intervention by other members of the interdisciplinary team, as appropriate.

  • Initiates communication with the attending physician, other interdisciplinary staff members and other agencies to coordinate optimal care and promote teamwork.

  • Performs therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician.

  • Performs clinical assessment of death, provide support to the family, coordinates with funeral home of choice and prepares patient for family prior to transport.

  • Performs initial assessment of individuals referred for care, determine eligibility using Hospice criteria.  Performs admission of the patient as appropriate.

  • Maintains an accurate clinical record through the timely preparation and documentation of care plans and clinical and progress notes utilizing (EMR).

  • Maintains regular communication with members of the interdisciplinary team, alerting attending physician and assigned Case Manager to any changes in the patient’s condition or needs as well as potential problems or issues.

  • Provides effective teaching to family, caregivers and interdisciplinary team members to ensure continuity of care for the patient.

  • Maintains familiarity with Federal, State, and Hospice policies and procedures.

  • Accepts responsibility for personal professional growth/development through participation in continuing education programs, agency in-services and community events.

  • Adheres to work schedule and maintains good punctuality and attendance record
  • Promotes hospice philosophy.
  • Explains hospice services and benefits to patients, family members and significant others.
  • Obtains signatures on all required forms, provides copies to patient/responsible party.
  • Makes appropriate referrals for other discipline services, medical equipment and pharmaceutical needs, i.e., pain/symptom management, DME, meds, supplies, services.
  • Performs other duties as assigned.   
  • Participates in weekend, holiday and on call coverage for admission needs as necessary.

SECONDARYFUNCTIONS:

All otherduties required by immediate supervisor.

QUALIFICATIONS:

Education/Skills                                    Graduateof an accredited Diploma, Associate, or Baccalaureate School of Nursing

Certification/Licensure                      Must be aregistered nurse licensed in the State of branch location, or in

                                                                accordancewith the Board of Nurse Examiners rules for Nurse Licensure

                                                                Compact.  Must have a valid and current Driver’sLicense and vehicle

                                                                insurance.

Training                                                  Twoyears experience as a Registered Nurse in a clinical setting, hospice preferred.  Must complete a minimum of 20 hours ofcontinuing education within each 24- month licensure period.  Must complete CIMA’s on the job internshiptraining with an assigned mentor utilizing agency specific competencymodules.  Employee will function underthe supervision of a mentor until competency has been demonstrated in allappropriate areas.  

Special Requirements                         Strongleadership and organizational skills. Excellent verbal and written communication skills.  Exhibition of good judgment and ability towork independently. Must possess a motor vehicle in good working condition,which is available during work hours, with current proof of insurance.  Must agree to and pass a criminal historybackground check.

Interrelationships                               Hospicephysicians, hospital and/or nursing home staff, referring physicians, and theirstaff, Hospice IDT members.
