Job Description

ESSENTIAL FUNCTIONS:

  • Responsible for directing and providing the nursing care for hospice patients in their place of residence, ages 16 years and older.  Coordinate care with the assigned interdisciplinary team, patient/family and referring agency. 
  • Assist patients and their families with the inevitable symptoms of a terminal illness, improve the quality of life of a patient's last days and to offer comfort and dignity,  high quality healthcare to those who are facing terminal illness, coping with grief or seeking to learn more about these profound experiences.
  •  The interdisciplinary team of physician's, nurses, certified home health aides, chaplains, social workers and counselors help patients and families prepare for, understand and make informed decisions at the end of life and throughout the grieving process.


Duties include: Case Managing hospice patients, attending interdisciplinary team meetings, plan of care updates, updating medications and medication adjustments, supervising any LPN or HHA that assist the RN in patient care, back up On Call. Must have good transportation and be able to work with computers and other documentation devices.

QUALIFICATIONS:

Education/Skills

 

Graduate of an accredited Diploma,Associate, or Baccalaureate School of Nursing

Certification/Licensure          Must bea registered nurse licensed in Texas, or in accordance with the

Board of Nurse Examiners rules for Nurse LicensureCompact.  Must have a current TexasDrivers License.

Training

 

Two years experience as a RegisteredNurse in a clinical setting, hospice preferred. Must complete a minimum of 20 hours of continuing education within each24- month licensure period.  Must completeon the job internship training with an assigned mentor utilizing agencyspecific competency modules.  Employeewill function under the supervision of a mentor until competency has beendemonstrated in all appropriate areas.  

 

SpecialRequirements

Strong leadership and organizational skills.  Excellent verbal and written communicationskills.  Exhibition of good judgment andability to work independently. Must possess a motor vehicle in good workingcondition, which is available during work hours, with current proof ofinsurance.  Must agree to and pass acriminal history background check.
